RENTALSA Request for Facility Use Form must be completed at the Dublin Memorial Park Recreation Facility. All rental agreements must be paid for in advance and approved by the Recreation Director prior to any scheduled event. If payment is not received in an appropriate time, events may not be adequately prepared. Checks must be payable to the City of Madison.
