Requests for Public Records
How to Request Public Records
To make a request, do the following
  1. Pick up or download the Request for Information Form
  2. Fax the completed form to (256) 772-5668, email it, or mail it to:
    City Clerk
    100 Hughes Rd.
    Madison, AL 35758

Applicable charges for copies and/or hourly rate for personnel to provide services are as follows:
  • Misc. Photocopies (if less than 15 minutes to prepare) - $0.25 per page
  • Misc. Photocopies (over 15 minutes to research and prepare) -  $0.25 per page plus rate of labor billed in 1/4 hr. increments
  • See page 2 of the "Request for Information" form hyperlinked above for any additional fees for large format copies and various department specific documents
