How to Request Public Records
Submit your request. Allow for a sufficient and reasonable time period for records to be made available. The time will include making available staff that may be limited due to performance of critical day-to-day work assignments. Research fees will apply for any request that takes longer than 15 minutes to complete.
Fees (effective March 1, 2012 per Ordinance 2012-42)
Applicable charges for copies and/or hourly rate for personnel to provide services are as follows:
- Misc. Photocopies (if less than 15 minutes to prepare) - $0.25 per page
- Misc. Photocopies (over 15 minutes to research and prepare) - $0.25 per page plus rate of labor billed in quarter-hour increments

Economic Development Business Survey
To better provide economic development activities that support your businesses, the Madison Economic Development Department needs to collect some data from you with your licensing application. This data enables us to analyze current trends, opportunities, and best methods to assist business within the City. It also provides required information so we can apply for grants and provide financial assistance for business expansion and growth.

Business License and Tax Application
- Click here for a new business license and tax application. It may be submited through the pdf form and we will get in contact with the applicant regarding payment or other information that may be needed. All revenue forms and applications work best when filled out and submitted on a computer rather than a phone unless you have the free Adobe Acrobat app.
Online Tax Filing
- Sales and Use Tax, Rental Tax, and Lodging Tax filing may be done online through the My Alabama Taxes (MAT) website. Please be sure to have your Tax ID number. If you do not know it, please contact us before filing online.
Tax Forms
- Tax Forms are now fillable and can be submitted online. The SUBMIT button at the bottom of the form will email a copy to the revenue department. If you plan on filing a zero tax return, please use this new feature to submit online.
